- class description and details
- phone/email addresses for co-teachers, substitute teachers, and/or specific TA's you would like to request
• You will not be allowed to advance if a required section is left blank.
• You cannot return to the form to make changes once the form is submitted. You may contact your Team Leader or Class Director if any changes are needed.
• You may enter comments/questions/feedback at the end of the form.
• You will receive an email of your class description upon submission.
